Who should complete this application?

The application is for students who are, for the first time, seeking disability services or accommodations at UNLV.
This application is also for returning UNLV students who wish to reactivate their DRC file and obtain disability Services.

DRC Application Process:

Complete all items below and submit the form electronically. This will generate an email to the DRC and inform us of your interest.
Provide the DRC with medical, psychological or psycho-educational documentation that supports your request. You may fax, mail, or hand deliver the documentation.
Upon receipt of your application and the supporting documentation the DRC will conduct a review your application. Reviews are conducted once per week throughout the year.
You will be contacted once the review is complete regarding your eligibility.

If your application is approved the DRC will conduct an intake meeting with you
If your application is not approved the DRC will inform you of what may be needed or what your options may be.

If you are not diagnosed with disability or if you currently do not possess documentation please contact the DRC office 702-895-0866 and request a meeting to talk with the Director.
